Transition
The ultimate goal of the Washtenaw Intermediate School District (WISD), Special Education Department is the successful transition of all students from school to post-school activities - whether postsecondary education, vocational training, integrated employment, continuing and adult education, adult services, independent living or community participation.
Transition planning is the foundation for IEP planning process. The purpose of this website is to assist all stakeholders (students, teachers, families, and other school personnel) in developing and implementing the Transition IEP and transition planning throughout the secondary years.
Transition Library
The Transition Library is a practical toolbox created to assist teachers with post-secondary planning. Developed by the WISD Transition Teacher Consultants, it categorizes age appropriate assessments/surveys/inventories and planning tools based on four main variables; the domain topic area you are assessing (education/training, employment, independent living, and community participation), school age level of the student (middle school, high school, and post-high school), and disability.
There are several transition tools in the transition library including assessments, surveys, inventories, templates, and educational resources/guides to assist in the transition planning process. The library includes assessments/surveys/inventories and other transition planning tools which are downloadable, available to complete online, and those of which are available to checkout through the WISD Destiny Library. We know that our students have various needs and that different and multiple assessments/surveys/inventories are needed to individualize the transition planning for our students. Therefore, our transition library includes varied tools from other educational institutions/entities, colleagues, and other transition professionals. It is important as an educator that you and the team choose the right tools for assessing and transition planning for your students.
Transition Tips
Transition Tips are shared weekly by email and accessible on the WISD website for teachers, paraprofessionals, related staff providers, administrators, parents, counselors, young adult transition agencies and vocational rehabilitation staff. Transition Tips cover laws/rules/regulations, and compliance with regards to transition, assessments/inventories/surveys, national transition practices, strategies, transition planning, work-based learning, academic and life skill instruction, family involvement, professional development offerings, student offerings/opportunities and information about and how to connect with postsecondary/adult transition agencies.
